When a tooth needs to come out

We take a tooth out only when it cannot be saved, or when keeping it would damage the teeth around it.

In practice that means severe decay that has destroyed too much of the tooth, a fracture running below the gum, advanced gum disease that has taken the supporting bone, infection that a root canal cannot resolve, or wisdom teeth that have no room to come through.

Occasionally teeth are removed for orthodontic reasons — to create space where the jaw is too crowded to align teeth otherwise. And sometimes a baby tooth is stubbornly holding up the adult tooth behind it.

If a tooth can reasonably be saved, we will tell you so and explain what saving it involves. Extraction is straightforward and often cheaper in the short term, but every gap has consequences: neighbouring teeth drift, the opposing tooth over-erupts, and the bone in the gap begins to shrink within months.

Wisdom teeth

Wisdom teeth arrive in the late teens or early twenties, usually into a jaw with no room left for them. When they come through cleanly and you can keep them clean, they can stay. Problems appear when they are impacted — stuck at an angle against the tooth in front, or only partly through the gum.

Partly erupted wisdom teeth are the troublesome ones. The flap of gum over them traps bacteria and food in a place no toothbrush reaches, which causes recurring infection, and the pressure against the neighbouring molar can decay a perfectly healthy tooth from behind.

That is why removal is often advised before symptoms rather than after — younger roots are less developed and recovery is markedly easier at twenty than at forty.

Simple and surgical extractions

What to expect, step by step

1. Examination and x-ray

   We look at the root shape and how close it sits to the nerve and sinus. This is what tells us whether the extraction is simple or surgical, and we explain which before we begin.

2. Numbing

   The area is fully anaesthetised. You will feel firm pressure during the extraction — that is normal and expected — but you should not feel pain. Tell us at any point and we will add more.

3. Removing the tooth

   The tooth is loosened and lifted out. Surgical cases may need a small incision or the tooth sectioned into pieces, which is often gentler on the surrounding bone than forcing it out whole.

4. Protecting the site

   The socket is cleaned and, where you may want an implant later, a bone graft can be placed at the same appointment to preserve the ridge. Doing it now is far simpler than rebuilding it later.

5. Aftercare instructions

   Gauze, written instructions and a number to call. We would much rather you rang with a question than worried at home.

Your recovery, day by day

Knowing what normal looks like makes recovery far less worrying, so here is the usual shape of it.

Day one. Bite on gauze for a solid twenty minutes without lifting it to check. Some oozing for the first several hours is expected — saliva makes a very small amount of blood look dramatic.

Use a cold compress on the cheek in twenty-minute cycles, keep your head elevated, stick to cool soft food, and take pain relief before the anaesthetic wears off rather than after. No rinsing, no straws, no smoking.

Days two and three. Swelling usually peaks around day two, which surprises people who expect steady improvement — it is normal, not a setback. Bruising on the cheek or jaw can appear now and is harmless. Start gentle warm salt water rinses after 24 hours. You can switch from cold compresses to warm ones from day two, which helps the swelling disperse.

Days three to seven. Discomfort should be clearly easing. This is the window when dry socket announces itself, so pain that suddenly worsens around day three, often with a bad taste and an ache spreading to the ear, means call us — it is straightforward to treat and there is no reason to endure it. Otherwise, jaw stiffness is common after a surgical extraction and gradually resolves.

Week two onward. The gum closes over, any dissolving stitches disappear, and you return to normal eating. The bone underneath continues filling in quietly for several months. Call us if pain worsens after day three, if bleeding will not settle, or if swelling comes with a fever.

Cost depends on whether the extraction is simple or surgical and how many teeth are involved. We x-ray and examine first, then give you a written estimate before anything is scheduled.

Extractions are classed as necessary treatment, so most PPO plans contribute, and Medi-Cal / Denti-Cal covers extractions for eligible patients. We verify your benefits before treatment. If you have no insurance, tell us when you book — financing is available and nobody should be living with an infected tooth because of cost.
