How professional whitening works
Whitening gel uses hydrogen or carbamide peroxide to break down the stain molecules that have built up inside your enamel over years of coffee, tea, red wine, tobacco and simply getting older. It does not strip or damage the tooth surface — it lifts colour from within it.
The difference between professional whitening and a drugstore kit is concentration and fit. We use a stronger gel safely because your gums are protected and the trays are moulded to your teeth, so the gel sits where it should. A generic tray leaks, which is what causes the patchy results and the burnt gum edges people complain about.
In-office or take-home?
| In-office | Custom take-home trays | |
|---|---|---|
| Time to result | One appointment | 1–2 weeks of nightly wear |
| Gel strength | Highest, applied by us | Lower, worn longer |
| Sensitivity | More likely, short-lived | Gentler, easier to pace |
| Control | We manage it entirely | You stop at the shade you like |
| Topping up later | New appointment | Keep the trays, buy gel |
| Best for | An event with a deadline | Gradual change and long-term upkeep |
Many patients do both — an in-office session to make the jump, then trays at home to hold it. If you are choosing one, custom trays give better long-term value because you keep them for years.
What whitening will and will not lift
Whitening works best on yellow and brown discolouration from food, drink, tobacco and age. Those respond well and predictably.
It works less well, or not at all, on:
- Grey staining, particularly from tetracycline antibiotics taken in childhood — it may lighten a little, but expect modest change
- A single dark tooth after trauma or a root canal, which needs internal whitening or a veneer instead
- Existing dental work. Crowns, veneers, bonding and fillings do not change colour at all
- White spots, which can actually look more obvious immediately afterwards before evening out
That fourth point catches people out, so it is worth stating plainly: if you have a crown or a white filling on a front tooth, whitening will lighten the teeth around it and leave that restoration behind. Sometimes the right plan is to whiten first and replace the filling afterwards to match.
What to expect, step by step
Check-up first
We examine before whitening. Peroxide on an untreated cavity or through inflamed gums is genuinely uncomfortable, and any decay should be dealt with first.
Recording your starting shade
We match your current shade against a guide and photograph it. This matters — the change is gradual and people forget where they started.
Whitening
For in-office treatment your gums are isolated and gel is applied in timed cycles. For take-home, we scan for custom trays and show you exactly how much gel to use, which is less than most people assume.
Managing sensitivity
Some sensitivity is normal and temporary. Desensitising toothpaste in the run-up helps a great deal, and take-home trays can simply be worn every other night if you need to slow it down.
Reviewing the result
We compare against your starting photograph and plan any maintenance. Most people top up occasionally rather than repeating the whole course.
How long results last
Typically one to three years, and the range is mostly down to habits. Coffee, tea, red wine, dark sauces and smoking all pull colour back in. The first 48 hours after whitening matter most, as teeth are temporarily more absorbent — keep to lighter-coloured food and drink during that window.
Keeping up regular cleanings makes a real difference too, since surface staining and tartar dull the result independently of the internal colour. If you have custom trays, an occasional single night of gel restores the shade rather than starting over.
What whitening costs, and coverage
Whitening is one of the least expensive ways to change how your smile looks, and considerably cheaper than any treatment involving porcelain. Cost varies between in-office and take-home trays, and we will quote you at the consultation before anything starts.
Whitening is classed as cosmetic, so PPO insurance and Medi-Cal / Denti-Cal do not cover it. We will be upfront about the cost rather than let you assume otherwise. If you are considering wider changes, see our cosmetic dentistry page — whitening is usually the sensible first step, and often the only one needed.
Why drugstore whitening kits disappoint
Patients often arrive having tried two or three over-the-counter products and concluded their teeth simply will not whiten. Usually the teeth were never the problem.
The first issue is fit. A one-size tray cannot seal against your teeth, so gel leaks out onto the gums — which both wastes the active ingredient and causes the burning sensation people report.
It also pools unevenly, which is where patchy results come from. A custom tray scanned to your teeth holds a thin, even film of gel exactly where it is needed and nowhere else.
The second is concentration and contact time. Retail products are legally limited to low peroxide levels, so they rely on being worn for a long time to achieve very little. Whitening strips have a further limitation: they are flat, and teeth are not.
Strips make good contact with the front surfaces and poor contact with the curved edges, which is why teeth whitened with strips often look bright from directly in front and unchanged from the side.
Then there are the products that do not whiten at all. Charcoal toothpaste and many whitening pastes work by abrasion — scrubbing surface stain away rather than lifting colour from inside the enamel.
They can make teeth marginally cleaner while doing real damage to the enamel over time, and worn enamel exposes the yellower dentine underneath. Some of the most stubbornly yellow teeth we see belong to people who have been scrubbing them for years with the best intentions.

Frequently asked questions
How long does teeth whitening last?
Usually one to three years, depending on habits. Coffee, tea, red wine and smoking pull colour back fastest. If you have custom trays, an occasional single night of gel restores the shade rather than repeating the full course.
Does whitening damage your enamel?
No. Professional whitening lifts stain from within the enamel rather than stripping the surface. The risks come from ill-fitting drugstore trays leaking gel onto the gums, which is exactly what custom trays prevent.
Will whitening work on my crowns or fillings?
No — crowns, veneers, bonding and fillings do not change colour. If you have a white filling or crown on a front tooth, we usually whiten first and then replace that restoration to match the new shade.
Is teeth whitening painful?
Some temporary sensitivity is common and settles within a day or two. Desensitising toothpaste beforehand helps, and take-home trays can be worn every other night to make it gentler.
In-office or take-home trays — which is better?
In-office is fastest and suits a deadline. Custom take-home trays give more control and better long-term value because you keep them and can top up for years. Many patients do both.
Does insurance cover teeth whitening?
No — whitening is classed as cosmetic, so PPO plans and Denti-Cal do not cover it. We will quote you clearly at the consultation, and it remains one of the least expensive cosmetic treatments available.
Do you take Medi-Cal / Denti-Cal for this treatment?
Yes — we welcome Medi-Cal and Denti-Cal patients, and we also accept all major PPO insurance and cash. Bring your Benefits Identification Card and our team will verify what your plan covers before treatment.
